The incumbent is responsible for providing intensive community-based treatment services to this population.
Work includes monitoring consumer medications and overall physical health, care coordination with community providers, training in health and wellness skills and activities of daily living, social skills, interpersonal relationships, financial management, etc.
Position requires strong organizational and time management skills, excellent writing skills, flexibility, and self-motivation.
Valid VA driver's license and acceptable driving record is required.
Starting salary of:
